New Client?

***Please complete the Intake Form and Teletherapy Permission Form listed below in order for us to process your child accordingly. Please fill out the Teletherapy Permission form even if you are not a virtual client. This form just keeps the option of virtual therapy open to you in the event necessary. 

To be evaluated at First Words, you will first need a recent hearing test (last 6 months) and a school IEP (if applicable). Please be aware,  if you cancel your initial evaluation less than 24 hours in advance, you will be put on a waiting list. It may take 2-4 weeks to reschedule. We will contact you within 2-3 days to follow up.

Complete el formulario de permiso de teleterapia incluso si no es un cliente virtual. Este formulario solo le mantiene abierta la opción de terapia virtual en caso de que sea necesario.

Para ser evaluado en First Words, primero necesitará una prueba de audición reciente (últimos 6 meses) y un IEP de la escuela (si corresponde). Tenga en cuenta que si cancela su evaluación inicial con menos de 24 horas de anticipación, se lo colocará en una lista de espera. La reprogramación puede demorar de 2 a 4 semanas. Nos comunicaremos con usted en un plazo de 2 a 3 días para realizar un seguimiento.
